It is school district policy that any volunteer working with children at a Philadelphia public school obtain two security clearances every other year (it is time to renew your clearances if you did yours before 2007!).  This page will guide you through what you need to do.
 
Getting security clearance so that you can volunteer at Greenfield is actually an easy and quick process.  Follow these easy steps, and shortly, you will be able to volunteer at Greenfield!
 
STEP 1)  State Police Criminal History Request  
 
You can complete this online, using a credit card to pay the $10 fee. Click on the "PATCH system" hyperlink.  You will get your results almost instantly.  Print out the results - one for your file and one to bring to Greenfield.

You can also download the .pdf, print it out and complete the form and mail it in with a $10 certified check or money order.  Money orders are available at the USPS office at 2100 block of Chestnut.  You must pay with cash or a debit card.
 
STEP 2)   PA Department of Public Welfare, Child Abuse Clearance History Form (CY 113)
 
 
This form must be printed out and filled in 
 
Under "Purpose of Clearance" select the School box. 
 
You will need to get a $10 money order for this form.  Money orders are available at the USPS office at 2100 block of Chestnut.  You must pay with cash or a debit card.
 
Make a copy of both the completed form and money order for your file, one for Greenfield and the original which you send to the DPW on the address listed on the form.
 
STEP 3)  Bring all the paperwork to the Greenfield Main Office (marking your forms "parent volunteer").
  • a print out of your Criminal History Report
  • a copy of both your Child Abuse Clearance History Form and the money order
